Use this page to submit an EPH applicant’s information to Salesforce.

Please note:

  1. The prospect will not receive an auto response email from Salesforce after you submit this form. Make sure to follow up with them and provide the instructions for submitting transcripts and paying the application fee.
  2. The only required fields are first name, last name, and email address. These fields are the bare minimum needed to make a new Salesforce EPH record.
  3. Be sure to select “yes” on the email opt in question if you have gotten permission to add the applicant to our email list.
  4. Use the Questions/Comments field to include any notes on the applicant that you want captured in Salesforce.
