The following information is provided as an aid to the event coordinator(s) in dealing with the well-being of the participant.







I understand that my son/daughter is required to maintain health insurance. I understand that I am responsible for payment of any outstanding or unpaid medical charges.




Parent Permission and Medical Emergency Authorization

Parent Permission and Release of Liability
I hereby give my son(s)/daughter(s) named previously permission to participate in the event(s) listed. Although the event coordinator(s) will use the utmost precaution in guarding the health of the above participant(s) and preventing accidents, I release them from any liability in case of illness or injury as a result of this activity. Furthermore, I release the owner and driver of the car transporting my child(ren) to and from the event(s), from any liability in case of illness or injury.

Medical Emergency Authorization
In case of sudden illness or an accident to the above named participant(s) requiring immediate treatment or surgery while he/she is a participant in this activity, I authorize the chaperone(s) to take such action as seems appropriate to protect the health and physical well-being of the above participant(s). This authority extends to any physician(s) and/or surgeon(s) selected by the event coordinator(s) to perform medical and/or surgical procedures including examinations and tests necessary to preserve the health and physical well-being of the above named participant(s). All efforts will be made to contact the parent(s) or guardian(s) in case of emergency.





Code of Conduct

Youth Program Code of Conduct
The primary purpose of the Youth Program Code of Conduct is to ensure the safety and well-being of all participants at events and activities hosted at Rutgers, The State University of New Jersey. It applies to all participants including minors, their parents, and volunteers.

As a participant in this program, I will:
  • Conduct myself in a courteous manner and treat members, parents, volunteers, staff and others with respect. Appropriate language and behavior are expected at all times.

  • Respect and adhere to the rules and guidelines of the program including all those specific to this event or activity.

  • Uphold an individual's right to dignity by supporting an environment of inclusion which welcomes involvement of participants from all backgrounds.

  • Obey local, state and federal laws.

  • Conduct myself in a safe and orderly manner for the safety of myself, other campers, and staff. Campers exhibiting inappropriate, and/or unsafe, behavior may be asked to leave the program at the discretion of the Program Directors, without refund.

  • Exercise responsible, ethical behavior when using any camp materials and supplies, including, but not limited to, sports equipment, classroom supplies, lab equipment, technology devices, computers, games and network resources. Sharing your user name and password with others is not permitted.

  • Understand that my own electronic devices (including, but not limited to phones, games, computers) are allowed at camp, but that bringing such items is at my own risk. Rutgers Tech Camps is not responsible for any loss from theft, damage or otherwise. All devices must be shut off when a teacher, or counselor, asks. Phones should always be on silent mode, and making calls, browsing the web, using apps, or sending text messages, is prohibited without first asking your counselor.

Participants who fail to adhere to the Youth Program Code of Conduct are subject to a range of disciplinary actions. When appropriate, immediate corrective action will be taken at the event to ensure the safety and welfare of all participants.


I understand if I fail to adhere to the above Code of Conduct, I will be subject to disciplinary action and potentially prohibited from attending and participating in this event and future programs at Rutgers University.



Internet Consent and Waiver Form
The following information must be read by the camper and parent or legal guardian.

Rutgers Tech Camps are presented in partnership with Fleming Tech Camps.

The camper, and his or her parent(s) or guardian(s) must understand that Rutgers University (RU) and Fleming Tech Camps (FTC) make no warranties with respect to the Internet service, and  specifically assume no responsibility for:

1. The content of any advice or information received by a camper from a source outside of RU and FTC or any costs or charges incurred as a result of seeing or accepting such advice.

2. Any costs, liability, or damages caused by the way the camper chooses to use the RU / FTC network.

3. Any consequences of network service interruptions or changes.

By signing the Consent and Waiver Form, I (camper) agree to the following terms:

1. I will not use RU / FTC's network for illegal purposes of any kind.

2. I will not use the RU / FTC's network to transmit bullying, hazing, threatening, obscene, griefing or harassing materials. RU and FTC will not be held responsible in any way if I participate in such activities and I may be expelled without refund if I participate in this type of activity.

3. I will not use the RU / FTC network to interfere with, or disrupt, other network users, services or equipment. Disruptions include but are not limited to disruption of unsolicited advertising, propagation of worms or viruses, deleting or defacing files, and using network to make unauthorized entry to any other machine accessible via the network.

4.  I understand that the use of the RU / FTC network is a privilege and not a right, and that inappropriate use of the RU / FTC network will result in the immediate cancellation of my privilege to use it, and may cause expulsion from the program without refund.

5. I will not use the network to download, or upload, any pirated or unethical, software, apps, music, games, images, videos or other media.

Camper: By signing this consent and waiver form, I and my parent(s) and guardian(s) agree to abide by the restrictions below. I have discussed these rights and responsibilities with my parent(s) or guardian(s).

Parent(s) or Guardian(s): I have been advised that Rutgers University (RU) and Fleming Tech Camps (FTC) do not have control of the information available through the Internet or other electronic data sources. Sites accessible via Internet or other electronic data sources may contain materials that are illegal, defamatory, inaccurate, obscene, profane, sexually explicit, or potentially offensive to others. While RU / FTC's intent is to make internet access available to further the goals and objectives of its programs, campers will, without sanction of RU / FTC, have the ability to access materials that are not part of the program curriculum via the internet and world wide web.

Media Policy and Release
Rutgers, The State University of New Jersey routinely promotes programs and activities involving minors through various media. This includes, but is not limited to newsletters, newspapers, brochures, and displays. In doing so, the names and photos of members may be included to help tell the story.
