If Paying by Check, Money Order, or Authorized Purchase Order:
Proof of payment MUST be presented with this form to process your registration. You have two options:
1. By MAIL: Include your check, money order, or purchase order with your completed registration form. (Make checks payable to Rutgers, The State University of New Jersey.) Mail to: Rutgers OCPE, Attn: Registration Dept, 102 Ryders Ln, New Brunswick, NJ 08901-8519.
2. By EMAIL: Attach a completed registration form to your email and send to email@example.com. If paying by Purchase Order, attach official PO document to the email as well. If paying with check or money order, attach a photo or scan of the payment to the email. Mail the check/moneyorder before the class begins to our office: Rutgers OCPE, Attn: Registration Dept, 102 Ryders Ln, New Brunswick, NJ 08901-8519.
